  • Contact details: Including address, email address, and phone number, to communicate with you regarding consultations previous and up-coming.

  • Date of birth: To ensure you meet the minimum age requirement for services offered and/or if you will require a guardian present.

  • Health history and relevant medical information: To assess your physical condition and tailor our services to suit your specific needs, goals and to maintain your well-being.

  • Information related to personal preferences, requirements and needs: To further offer personalized and effective consultation sessions.

  • Scheduling and managing appointments: To book suitable time slots and provide confirmation details.

  • Providing personalized and effective services: To deliver consultations tailored to your individual requirements and preferences.

Booking and Cancellation Policy


Booking and Cancellation Policy for MEND Bodyworks

Effective Date: 1/7/2025


At MEND Bodyworks, to help us accommodate our clients and staff effectively, we kindly request that you notify us at least 24 hours in advance if you need to reschedule or cancel your appointment.
 

Rescheduling or Cancellation Fees:

  • With Less Than 12 Hours’ Notice: A fee equal to 50% of the service cost will be charged.

  • With Less Than 2 Hours’ Notice: The full service fee will be charged.

  • No-Shows: If you do not arrive for your scheduled appointment, the full service fee will be charged.

MEND Bodyworks reserves the right to void the above fees, for any reason. For example: in the case of emergencies or special circumstances.
 

Avoiding Late Notice Fees: All appointments can be transferred to another client to avoid any fee
 

Punctuality: Please ensure you arrive on time for your appointment. For all first time appointments, it is generally recommended to arrive 5-10 minutes early to complete some on-boarding forms for your consultant to use during your session.  If you arrive late, the consultation time may (at the discretion of the consultant) need to be reduced to avoid inconveniencing any next scheduled clients, without reducing the cost of the initial appointment booked.
 

Policy Enforcement: In the event of a cancellation which incurred a fee, the cancellation fee will be applied to your account and an invoice will be sent out to the contact details provided, which must be settled before booking any future appointments with MEND Bodyworks.
